I have developed a new P. Maybe I am missing something basic, but I am currently out of ideas. I'm confused by you programming header. The square pad usually denotes pin 1. Based on the power traces it appears rotated degrees. Read AVR Routing them out to some pads cost nothing except a bit of time and can make modifications a lot easier. A lot of wire's are going directly from the uC to connectors. Leaving inputs of an opamp U3 open is bad practice and can lead to oscillations and nasty stuff.
Besides of programming pins. I also emphasis ground plane, bypass cap as close as possible for U1, U4 Heat sink for U4 pad 2 protection diodes for U4.
